Corewell Health South is in the process of collecting input for the 2025 Community Health Needs Assessment (CHNA).

Below, you will find an explanation of its impact potential for our county and explore opportunities for collaboration in our efforts to engage with the CHNA. Please find the link for the survey here: CHNA Survey

Why is the Community Health Needs Assessment Important?

· The Community Health Needs Assessment helps our health system gain a deeper understanding of what the current priority health needs and aspirations are in Southwest Michigan.

· This helps Corewell Health South and community stakeholders understand what our community needs are and how we can allocate resources to address those needs.

· Feedback provided through the CHNA helps us in developing strategies & ways to address the priority health needs.
