Community Health Survey

Through a community health assessment, completed earlier this year in partnership with Spectrum Health Lakeland as well as many other partners, the community said that mental health and obesity are two of the biggest health issues facing residents in Berrien County.

As we move forward from assessment to action, we need your help, your voice and your ideas as a community leader & member to ensure that strategies and actions identified represent community desires and to ensure actions will lead to positive and impactful community change.

In our current COVID-19 reality and needs for continued social distancing, we have included a short survey in which to collect community resident and partner feedback.

This survey will take about 5 minutes and will be available until August 7, 2020.

Responses provided will drive action planning for the upcoming year, and support more equitable and healthy communities in which we live, work, play and raise our families.
